Social Worker-Case Management - FT and Per Diem
Assesses, provides appropriate counseling & intervention, and educates patients, families and significant others requiring emotional, social and psychological assistance. Provides care management, plus community information and referral, including multi disciplinary input. Geriatrics is the primary population served.

EDUCATION and/or EXPERIENCE: Master of Social Work degree and one year of clinical experience in medical setting preferred.
LANGUAGE SKILLS: Ability to communicate effectively with patients, families, patient care team members, referral sources and hospital management. Ability to understand basic medical terminology and interpret corresponding psychosocial and discharge planning needs. Able to write reports about patient treatment and business correspondence.
Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the community.
MATHEMATICAL SKILLS: Ability to add, subtract, multiply, and divide.
REASONING ABILITY: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Ability to handle numerous complex situations and prioritize activity.
